Adoption Agreement

Please read and respond to each item below.

I understand that there is an adoption fee and that it helps cover veterinary care, spay/neuter, vaccinations, and other rescue expenses.

I understand that AHCR warrants that it has NO knowledge of any vicious or destructive tendencies of this pet. I agree to assume full responsibility and liability for the actions of this pet once the adoption is finalized.

I agree to provide proper veterinary care, including annual vaccinations, heartworm prevention, flea/tick prevention, and prompt medical attention when needed.

I understand that the pet must remain spayed/neutered. If the pet is not yet altered at the time of adoption, I agree to have the procedure completed as directed by AHCR.

I agree to keep the pet as an indoor pet with outdoor access only under supervision or in a securely fenced area. I agree to keep AHCR dogs leashed at all times when outside of a fenced area.

I agree to return the pet to AHCR if I am no longer able to care for it. I will NOT rehome, sell, give away, or surrender the pet to any other person, shelter, or rescue without prior approval from AHCR.

I understand AHCR is not liable for any illness, disease, injury, or damage a person or pet may contract or sustain from the adopted pet.

Should any damages occur against my existing pet(s) from the adopted pet, I understand that I am responsible for any resulting veterinary costs.

I agree to provide the pet with a nutritious diet, fresh water, shelter, regular exercise, and socialization appropriate for the pet's needs.

I understand that AHCR reserves the right to conduct follow-up home visits and welfare checks after the adoption is finalized.

I agree to contact AHCR immediately if the pet is lost, stolen, or escapes.

I agree to comply with all local, state, and federal laws regarding pet ownership, including licensing and leash laws.

I understand that misrepresentation on this application may result in denial of the application or revocation of the adoption.

I understand that completing this application does not guarantee approval. AHCR reserves the right to deny any application.

A representative of AHCR has permission to inspect my home and property at a mutually convenient time prior to and/or after the adoption.

If it is determined by either party that the terms of this agreement cannot be fulfilled, I agree to return the pet to AHCR immediately.
